Our Adult Day Training program empowers individuals to thrive and lead fulfilling lives by building essential independence and life skills. Participants engage in hands-on activities that foster self-care, meal preparation, personal hygiene, and health management, alongside targeted support to enhance communication, social connections, and emotional coping mechanisms. Through personalized guidance, we strengthen self-advocacy by helping participants express their choices and exercise their rights, while also cultivating key pre-vocational habits like task completion, teamwork, and punctuality to prepare those who desire a path toward community employment.
The central coordinating hub for a participant’s care, ensuring they can access necessary Medicaid waiver services, medical state plans, and local community resources. A dedicated, conflict-free case manager conducts ongoing assessments of the participant's physical health, mental well-being, and social environment to build and maintain a custom, Person-Centered Service Plan (PCSP). Beyond coordinating standard agency services, case managers play a vital role in monitoring the health and safety of the individual, advocating for their needs during crises, and providing essential administrative guidance for families.
This service provides hands-on assistance, promptings, or repetitive, ongoing guidance with essential tasks such as bathing, dressing, grooming, eating, and personal hygiene, alongside necessary instrumental tasks like meal preparation, laundry, and light housekeeping. Delivered primarily within the participant's home.
A vital support service designed to provide short-term, temporary relief to a participant's primary, unpaid caregiver. Recognizing the immense physical and emotional demands of full-time caregiving, this service ensures that the participant continues to receive safe, high-quality care in their own home or an approved community setting while the primary caregiver takes a necessary break to rest, attend to personal matters, or handle emergencies.
Provides a structured, safe, and social environment during daytime hours for participants aged 21 and older who require a higher level of medical oversight. Operating out of The Marion House Adult Day Health Care Center. This service blends routine personal care, social activities, and nutritious meals with skilled nursing interventions. It is designed to safely manage a participant's complex health conditions or physical health care needs outside the home, simultaneously allowing them to engage with peers while giving their primary caregivers necessary daytime respite.
